- I authorize and direct One Down Dog LLC to charge my credit card on file for the payment of class fees and all other charges.
- I understand that if I opt for the monthly autopay, the amount will be charged to my credit card each month for a minimum of X (agreed length of contract) months.
- If my account goes into default (if unpaid 5 days past due), there is a $15.00 admin charge.
- I understand that should I make an invalid chargeback, I will be assessed a $25 admin fee.
IF I WISH TO DISCONTINUE AFTER MY CONTRACT IS FULFILLED, NOTICE MUST BE GIVEN IN WRITING AND SUBMITTED HERE AT LEAST 30 DAYS IN ADVANCE. NO CHANGES WILL BE MADE TO AUTOPAYS AFTER MONTHLY CHARGE IS PROCESSED.
- There will be a $10 administration fee to pause an autopay (waived for 1st pause). Max of 3 mo per yr
- Pauses must be requested here with 30 day advanced notice.
- Membership can only be paused for a month at a time and suspended time will be added on to the end of the contract.
- There will be no fee for suspension for medical purposes with Doctors note.
- Should I wish to cancel before the contract has been fulfilled, there is a cancellation fee of 50% of the remaining amount due.
AFTER THE 3 MONTH COMMITMENT, I UNDERSTAND THAT MY ACCOUNT WILL AUTORENEW ON A MONTH-TO-MONTH BASIS.
Should I wish to cancel my membership after I have fulfilled my contract, I must provide 30 days written notice here.
